A session of my can be broken down into three phases; an overview of what's transpired in class since the last time I saw the student, then answering questions, completing unfinished work, studying for tests, starting essays, projects and whatever they need the most help with. Finally, I go over all we've done, double check to make sure there isn't anything else to do, put everything where it needs to go and clean up. If a student is having trouble staying organized or needs to catch up in their reading, we can use any extra time we have together on those or other things.
